Blepharoplasty Eyelid Surgery is one of the most frequently performed procedures for aesthetic correction. As we age, the skin around our eyes loses elasticity and facial muscles relax, causing both the upper and lower eyelids to sag or form “bags” around the eyes. Blepharoplasty surgery is designed to address excess eyelid skin and excess tissue that contribute to a hooded appearance, restoring a more youthful and refreshed look.
This surgery is popular among everyone who want to achieve a fresher, rejuvenated facial appearance. Upper blepharoplasty and upper eyelid surgery are common procedures for rejuvenating the upper lids by removing sagging eyelid skin.
How the Blepharoplasty Eyelid Surgery is Performed
Blepharoplasty eyelid surgery includes both upper and lower blepharoplasty procedures to address sagging skin, remove excess fat bags, and tighten relaxed muscles around the eyes for a more youthful appearance. This blepharoplasty surgery is tailored to each patient, with a personalized treatment plan. Typically, surgeons perform the operation under local anesthesia when correcting only the lower eyelids.Modern techniques in blepharoplasty prioritize fat preservation to avoid an unnatural ‘hollowed-out’ appearance.
Upper eyelid surgery
For upper eyelid corrections, the surgeon makes an incision along the eyelid crease, also known as the natural fold, to conceal the scar. This key anatomical landmark helps achieve a natural and aesthetically pleasing result. The surgeon removes excess skin and any herniated fatty sacs, addressing the soft tissues—including fat and muscle—that may have shifted from the eye socket due to aging, before suturing the incision.
Lower eyelid surgery
For the lower eyelid, the incision is made just below the lashes or, in certain cases, inside the lash line. Lower blepharoplasty or lower eyelid surgery may involve techniques to remove excess fat or reposition excess fat for a smoother contour, as well as removing sagging skin. The procedure addresses the soft tissues of the lower eyelid to improve both function and appearance, and the incision is closed with an aesthetic suture.
Blepharoplasty procedures typically take between 45 minutes to two and a half hours, depending on the extent of surgery required.
Patients are usually able to return home on the same day of the surgery, depending on the level of surgery required.
Blepharoplasty results typically last for 5 to 7 years before another treatment may be needed.

Preparing for Eyelid Surgery
Preparing for eyelid surgery, or blepharoplasty, is a crucial step in ensuring a smooth and successful outcome. Before your procedure, you’ll meet with an experienced oculoplastic surgeon or cosmetic surgeon for an initial consultation. During this appointment, your surgeon will review your medical history, assess the condition of your upper and lower eyelids, and discuss your goals for the blepharoplasty procedure. This is the perfect time to ask questions and set realistic expectations about what eyelid surgery can achieve.
To minimize the risk of complications such as blood clots or excessive bleeding, your surgeon will advise you to stop taking certain medications, including blood thinners and herbal supplements, well before your surgery. Smoking and vaping should also be avoided, as they can delay healing and increase the risk of surgical complications. Lifestyle changes, such as stopping smoking weeks before the surgery, are recommended to aid recovery from blepharoplasty. you are scheduled to have your procedure under local anaesthetic or general anaesthetic, it’s important to arrange for someone to drive you home and stay with you for the first night, as you may feel drowsy or unable to perform daily tasks.
Blepharoplasty recovery
Setting up your home for recovery is another key part of preparation. Make sure you have cold compresses ready to help reduce swelling, as well as any prescribed eye drops or medications to promote healing. Creating a comfortable recovery area will make it easier to rest and care for your eyes in the days following surgery.Recovery from blepharoplasty includes swelling, bruising, and dryness, usually managed with cold packs, head elevation, and eye drops.
You should avoid smoking and wearing contact lenses during your blepharoplasty recovery.Cold packs can reduce swelling but it is recommended that you keep your eye area dry for a few days to help with recovery. You should wear dark sunglasses for the first couple of weeks post-treatment to avoid irritation from the sun and wind.
Remember, while eyelid surgery can effectively remove excess skin and fat from the upper and lower eyelids, it may not address every sign of aging or achieve complete symmetry. Your surgeon will explain the benefits and limitations of the procedure during your initial consultation, helping you make an informed decision about your cosmetic surgery journey. You will receive blepharoplasty aftercare instructions from your surgeon, which will involve gently cleaning your eyes with prescribed eye drops or ointment.
Full healing after blepharoplasty takes longer than the initial recovery period, with refined results visible in 3–6 months.

Potential Complications of Blepharoplasty Procedure
Although eyelid surgery blepharoplasty is considered a safe and effective cosmetic procedure, it’s important to be aware of potential complications that may arise. Some patients may experience temporary issues such as bleeding or hematoma, which can lead to skin discoloration or, in rare cases, affect vision. Infection is uncommon but can occur, typically managed with prescribed antibiotics or, if necessary, further surgery.
Other possible side effects include dry eyes or watery eyes, which are usually temporary and can be relieved with prescribed eye drops. Some patients notice numbness or tingling in the eyelids or surrounding skin, which generally resolves within a few weeks. Blurred vision or double vision may also occur, but these symptoms are typically short-lived.
In some cases, patients may notice asymmetry or unevenness in the eyelids, which could require further surgery to correct. Scarring is usually minimal and hidden in the natural crease of the eyelid, but visible scars can occasionally develop. Persistent eye bags or puffiness may need additional procedures or lifestyle adjustments, while drooping eyelids (ptosis) might require further treatment.
Discussing these potential risks with your surgeon during the initial consultation is essential. By following your surgeon’s instructions closely before and after surgery, you can help minimize the likelihood of complications and support a smooth recovery. Understanding both the benefits and possible risks of blepharoplasty will empower you to make the best decision for your health and appearance.
Your surgeon will give you detailed information for the preoperative preparation a few weeks prior to your plastic surgery.
The most important advice for smokers is to stop smoking for at least 2 weeks (or 4 weeks and even more, if possible). Smoking prevents healing and increases the risk of complications. You should not smoke until the wounds heal well, at least two weeks after surgery.
You should stop taking aspirin or medicine containing aspirin, antiflammatory drugs and herbal supplements as they all increase the risk of bleeding. Your surgeon will advise you how long prior to the operation you should stop taking these medicines.
High blood pressure also needs to be well controlled during surgery. So if your blood pressure is elevated, you should discuss it with your surgeon.
It is extremely important to discuss your expectations with your plastic surgeon. This will allow him to assess whether they are achievable with the selected procedure. In some cases, two or three sessions may be needed to achieve the desired effect, and in others – it may be necessary to combine procedures for a better result.
You will be accompanied by an employee of ours to the clinic and where you are staying before and after the operation.
Blepharoplasty recovery typically involves a short healing process, with most patients experiencing improvement in swelling and bruising within the first week and returning to normal activities within two weeks. Key milestones include suture removal, reduction of swelling, and follow-up appointments to monitor healing.
After the procedure, the eyes are not bandaged. Antibiotic ointment may be applied to the surgical site to prevent infection and promote healing. Vision may be slightly blurred due to the local anesthetic being used. Therefore, you should not drive for the first 24 hours after surgery. The sutures can be removed after a few days, as the skin of the eyelids heals extremely quickly. Suture strips may be used to support the eyelids during the first week and are typically removed at a follow up appointment. Hematomas will decrease significantly within the first 7 days. Swelling accompanies each operation. If the patient experiences swelling around the cheekbones, it may persist for 1-2 months, but will certainly eventually be resorbed.
The operation itself is not painful and the measures described below aim to reduce the temporary swelling. Wearing a cold compress immediately after surgery and sitting in bed for the first night are helpful, but do not affect the end result. The eye may have a „watery“ appearance due to the swelling around the tear ducts, which naturally drain the tears in the nose. Due to the operation they swell and temporarily block. This is also temporary and usually lasts one day.
You can resume physical exercise after 2 weeks.
For the first 2 weeks, it is good to sleep on your back with your upper body slightly raised.
During the initial recovery period, patients should avoid wearing contact lenses to prevent irritation and support proper healing of the eyelid area.
Upon completion of your surgery, you will receive complete and detailed oral and written instructions regarding all aspects of your recovery.
Every surgical procedure involves certain risks and it is important to understand their nature before proceeding. You should discuss any existing medical issues with your surgeon. It is wise to avoid aspirin or anti-inflammatory drugs for 10 days before surgery, as this may increase the risk of bleeding.
Possible complications at the surgical site include infection, bleeding, delayed healing, or scarring. It is important to monitor for these issues and report any concerns to your surgeon promptly.
Infections are rare in this surgery and can be treated with antibiotic eye drops and ointment.
Watery eye (epiphora) – the tear ducts, which usually drain into the nose, will temporarily become blocked due to swelling. In addition, swelling can give the eye a „watery“ appearance due to fluid under the conjunctiva. This can take a week or two.
Ectropion – swelling after surgery can cause the lower eyelid to sag and push the eyelid away from the eyeball. It usually subsides when the swelling subsides and can be aided by massage maneuvers in the area. This problem can occur in older patients as well as exist preoperatively. There are operational procedures for its correction.
