What is gynecomastia?

Gynecomastia is the excessive development of the breasts in men, which may involve the accumulation of fat and / or overdevelopment of the gland. In most cases, the cause is a metabolic imbalance, but the condition can also be caused by hormonal problems (e.g a decrease in testosterone production) or as a side effect of estrogen treatment and / or steroid intake.  The condition is often characterized by a genetic predisposition. Gynecomastia can develop at any age and usually causes physical and mental discomfort.

What are the ways to treat gynecomastia?

In some patients, the tissue to be removed is predominantly glandular. This specific operation is called “Webster procedure” and consists of making a discrete incision around the edge of the areola, through which excess tissue is removed.

In other cases, the excess is mainly subcutaneous adipose tissue, the so-called. pseudo-gynecomastia. In these cases, the following approaches are possible:

  • Liposuction or BodyTite procedure – for patients with minimal accumulation of subcutaneous adipose tissue without ptosis / sagging / of the skin;
  • BodyTite in combination with „defining“ liposuction procedure for lifting soft tissues – for patients with minimal accumulation of subcutaneous adipose tissue and minimal ptosis /sagging / of the skin;
  • Liposuction in combination with BodyTite procedure for lifting soft tissues – for patients with significant excess of subcutaneous adipose tissue and minimal ptosis /sagging/ of the skin;
  • Liposuction and excision /removal/ of excess skin – for patients with  significant excess of subcutaneous fat in combination with significant ptosis /sagging/ of the skin ;

Patients are often presented with excess skin and subcutaneous adipose tissue, in combination with gland overdevelopment – in these cases, liposuction is recommended in combination with excision /removal/ of the excess skin, subcutaneous adipose tissue and glandular component.

What should I do before the operation?

During the consultation, you will discuss with the surgeon exactly how much tissue needs to be removed and what the risks of re-occurrence of the condition are. It will be specified what exactly the treatment will be – only liposuction, only BodyTite – radiofrequency tissue lifting, only excision (removal by excision) or a combination of two or more techniques.

If you have decided to undergo surgery, you need to make the following changes:

  • Stop smoking 2 weeks before and after surgery;
  • Stop taking medicines that contain aspirin or other NSAIDs, as well as vitamin E and any herbal supplements, 7 days before surgery;
  • You need to be healthy – an infection or other illness can interfere with the operation.
Aftercare and recovery

Recovery time varies, depending on the case and the specific procedures used. In general, you will be able to return to your normal daily activities in about two weeks.

 Fitness and sports activities should be stopped for 4 weeks. During this time, you should be careful about all actions that require raising the elbows above the shoulders.

 All surgical techniques that can be used require an incision to be made, which will result in a scar. It will be red and slightly swollen for a few months after the procedure, before fading completely.

 It is very important for the good appearance of the scars to follow exactly all the care instructions that will be given to you by your surgeon and his team. With certain genetic predispositions, a rough and uneven scar is possible to form, even with an aesthetically pleasing suture of the skin and subcutaneous tissue. Therefore, this should be discussed during the preoperative consultation.

What are the scars that remain after this operation?

The different procedures and techniques that can be used for breast reduction  lead to different scars, which are usually minimal. Most often you can expect scars around the nipple and / or in the lower part of the breast fold and / or a vertical scar that connects them.

 However, if only liposuction and / or BodyTite techniques are used for breast reduction, then the scars may be minimal.

What are the risks of a male breast reduction?

Scars – our surgeons will do their best to avoid visible scars, no matter what procedures have been performed. For best results, the patient will need to follow the postoperative scar care instructions exactly. However, there are some patients who are genetically predisposed to have problem traits and this is difficult to avoid, but can certainly be reduced.

Bleeding – it is a rare complication of Webster’s procedure. That is why it is important to stop all medications that increase the risk of bleeding before surgery.

Recurrence – any hormonal imbalance that is not treated properly will continue after surgery, so the tissue that remains can also increase in size. Of course, once a portion of the tissue is removed, the increase will not be so great, but it still exists as an option.

Swelling, bruising and pain – Bruising after breast liposuction is often greater than in other areas and lasts longer. This is not dangerous, but it requires patience.

Seroma – it is possible to collect „liquefied“ adipose tissue in the subcutaneous tissue, both in the early stages after the intervention and in the late postoperative period – 2nd to 3rd month. In most cases, these quantities are in a minimum volume – 20 / 30ml and are absorbed and disappear without the need for additional intervention. In rare cases, it may be necessary to withdraw the fluid by puncturing it, and if it is necessary to perform this type of intervention, it is relatively painless due to the subcutaneous localization of the fluid.

Increased and decreased sensation after surgery – most patients will experience some change in the sensation of their breasts, most often a temporary tingling sensation around the nipple.

Asymmetry – The appearance of asymmetry is possible when more tissue is removed from one of the breasts. It is usually noticed when a few weeks have passed since the operation.

Unsatisfactory results – Sometimes patients are not satisfied with the result of their gynecomastia, because the expectations and requirements are not in line with the technical capabilities. It is important to have a careful discussion with your surgeon in order to avoid this disappointment.

How do we work

Once you have made contact with us, we will organize a free video consultation with the surgeon we believe is best suited for your particular situation.

This consultation will be within 5 days of your request. Prior to the video consultation, we will ask you to fill out a questionnaire about your medical history and send photos of the problematic areas. 

If necessary during the video consultation your doctor will advise you to take additional photos of the problematic areas and suggestions for the angle from which to take photos.

This consultation will be combined with a detailed discussion about your wishes and the possibilities for achieving them.  We will discuss with you the expected results, possible risks and any other issues that you might be concerned about.

We advise you to make a list of all the questions you may want to ask concerning the procedure before the actual consultation. After the official request to us, and if you wish, we can arrange a meeting for you with our representative in London.

Within 24 hours of the video consultation, we will send you a costing describing the price of the procedure and the needed length of stay in Bulgaria. In the costing there will also be preferential prices offered for our partner hotels with which we work and for which we can guarantee the quality and comfort. 

You will then have 3-4 days of time to reflect, and give us your decision.

If you decide to proceed and agree with all terms and conditions , you will have 48 hours within which you must prepay 30% of the total amount of the procedure. This amount is paid directly to the clinic where your procedure will be performed and thereby you will have a reserved date and time for your procedure and a room in the hotel you have chosen. The remaining balance has to be paid by the day before the procedure itself after conducting the preliminary examination in the clinic.

If you decide or need to cancel the procedure, this must be done at least 30 days before the date of your flight in order to be able to trigger a process for returning your deposit.  

If you decide to cancel our services within the last 30 days before your flight, you must have a good reason for doing so (supported by evidence) in order to take your money back. If you cancel it within these 30 days and the reason is acceptable, your money will be returned, but you will be suspended 50 euro for an administrative fee. If you wish to change the date of the procedure, this can happen at least 30 days before.

If you wish, we can also arrange your travel from your home to the airport. We are partners with a transport company and you can use our preferential prices. This also applies to your return flight. To do this, you will need to send us the details of your flight one week in advance.

Upon arrival in Bulgaria, you will be greeted at the airport by one of our employees who will take you to your hotel.

On the day before the operation, you will have a preliminary examination, where some necessary tests will be performed (e.g. blood tests) and we will specify all the details related to the operation (treatment plan, proposal for some additional or adjacent procedures for a better outcome, recovery plan etc). Our employee will take you to the clinic and after that to the place where you are to be accommodated.

Before the surgery, you will need to sign an informed consent document (i.e. a document certifying that you have been informed by the doctor about all potential risks and complications and that you understand them) and agree to proceed with the procedure).

On the day of the operation our employee will bring you back to the clinic. Depending on the procedure, you may need to have one or several days of hospital stay. After the completion of the procedure and the hospital stay, our employee will take you back to your hotel.

During the entire stay, our employee will be at your service throughout.
